Where: Productivity → Automation → Questionnaires (or Settings → Tools → Questionnaires / Form Templates).
Form templates are the extra questions and forms candidates see when they apply—screening questions, consent checkboxes, or custom fields. Steps:
- Create a new template and give it a name.
- Add questions one by one: multiple choice, short text, long text, yes/no, etc. Mark which are required so the application can’t be submitted without them.
- Save the template.
- When creating or editing a job (Core → My Jobs → Create Job or open a job), attach this template to that job. Applicants will see the questions after the main application fields.
Responses appear on the applicant profile so you can filter and review. To reuse the same set of questions across jobs, use one template and attach it to each job.
